System Pharmacy Manager, Formulary and Drug Policy

Indrani Kar, PharmD, DPLA is the System Pharmacy Manager for Formulary and Drug Policy at University Hospitals Health System in Cleveland, Ohio. Dr. Kar works with interdisciplinary teams throughout the health system to coordinate formulary management of medication use that is safe, effective, and cost-conscious for patients and the health system. Additionally, she coordinates system policy and education to assist staff in practicing at the top of their license and facilitates interdisciplinary projects to address system initiatives. Dr. Kar supports the UH PGY2 Medication Use Safety and Policy Residency as RPD.
Dr. Kar received her PharmD from the University of Pittsburgh. She subsequently completed two specialized programs in drug information; a fellowship at the University of Missouri-Kansas City School of Pharmacy and a PGY2 residency at Rutgers, Ernest Mario School of Pharmacy, and Robert Wood Johnson University Hospital. Dr. Kar has served on the ASHP Medication Safety Section Advisory Group, as the new practitioner member for the Council on Therapeutics, and as the Chair of the Education Steering Committee for the ASHP Section of Inpatient Care Practitioners. She currently serves as the ASHP SICP Director at Large. She was recently honored with the Ohio Society of Health System Pharmacists (OSHP) 2022 Pharmacist of the Year award. Her interests include drug information, education/teaching, formulary/policy management, and interdisciplinary collaboration.
