Christine Formea PharmD, BCPS, FACCP, FASHP is an Associate Professor and Enterprise Pharmacogenomics Pharmacist for Intermountain Health in Salt Lake City, Utah. She completed her BS degree in Pharmacy at the University of Colorado Health Sciences Center in Denver and Doctor of Pharmacy degree at the University of Texas at Austin. She completed a Pharmacy Critical Care Residency at Detroit Receiving Hospital and a fellowship in Transplant Pharmacogenomics at the University of Florida at Gainesville. Dr. Formea is a Board Certified Pharmacotherapy Specialist and a Fellow of the American College of Clinical Pharmacy and the American Society of Health-System Pharmacists. Currently, she leads Intermountain Health implementation efforts for pharmacogenomics clinical services, education, and research programs with clinical practice partners, Ambulatory Care Pharmacists, and Intermountain Precision Genomics. She co-Chairs the recently launched Intermountain Pharmacogenomics Pharmacy and Therapeutics Subcommittee to translate and implement pharmacogenomics into clinical practice. Dr. Formea collaborates and expands pharmacogenomics knowledge with groups including CPIC, NHGRI ISCC-PEG, IGNITE, PMC, ASHP, ACCP, and STRIPE.
