Director of Rural Pharmacy, MTM and Anticoagulation

Todd Lemke, PharmD is currenly the Director of Rural Division Pharmacy, MTM and Anticoagulation with CentraCare in central Minnesota. He completed a PGY1 residency through the University of Minnesota after obtaining his PharmD degree from the same university. His practice involves caring for patients in both hospital and ambulatory patients settings, primarily in rural areas, as a clinical pharmacist and currently as rural division director. Directing the pharmacy departments for two critical access hospitals and disproportionate share hospital, he is also has system responsibilities for medication therapy managment, anticoagulation services and COVID therapeutics response teams. He is a residency site coordinator for the University of Minnesota and student preceptor for multiple colleges of pharmacy. Todd Lemke is active in ASHP on the Small and Rural Hospital SAG, and was recently appointed to the Council on Pharmacy Practice as well as being part of his state pharmacists organziations. In 2022, he was awarded the Bowl of Hygeia award.
