Nicholas Leon, Pharm.D., BCPS, BCACP, FCPP is an associate professor at the Thomas Jefferson University, Jefferson College of Pharmacy in Philadelphia, PA.
Dr. Leon was the first ambulatory care pharmacist to provide comprehensive medication management and chronic disease management services at the Penn Center for Primary Care, where he practiced for 13 years. Dr. Leon is the founding Chair of the Board of Directors for the Pennsylvania Pharmacists Care Network (PPCN), an organization that is among the largest and most active regional affiliates of the Community Pharmacy Enhanced Services Network (CPESN). Dr. Leon is also a founding member of the leadership team for the Academia-CPESN Transformation (ACT) Pharmacy Collaborative.
Dr. Leon is a past-president of the Pennsylvania Pharmacists Association (PPA). He is dual certified as a specialist in pharmacotherapy and ambulatory care by the Board of Pharmacy Specialties (BPS), and was elected into Fellowship at the College of Physicians of Philadelphia in 2017.
Dr. Leon is a graduate of the University of the Sciences in Philadelphia (now the Philadelphia College of Pharmacy at Saint Joseph’s University). He completed a PGY-1 pharmacy practice residency at the New Mexico Veteran Affairs Healthcare System in Albuquerque, NM and a PGY-2 ambulatory care residency at the University of Maryland, School of Pharmacy in Baltimore, MD.
