Transitions of Care Coordinator

Nancy MacDonald, PharmD, BCPS, FASHP is the Coordinator of Transitions of Care, Emergency Medicine Pharmacy Services and Emergency Preparedness at Henry Ford Hospital (HFH). She completed her Bachelor of Science in Pharmacy at Wayne State University and her Doctor of Pharmacy at Shenandoah University. She completed the American Society of Health-System Pharmacists (ASHP) Leadership Academy. Nancy's previous research focused on transitions of care, medication access, culture follow for emergency department patients, and advancing ambulatory care practice. She is the Past President of the Michigan Society of Health-System Pharmacists (MSHP) and a former Director for the Southeastern Michigan Society of Health-System Pharmacists (SMSHP) and MSHP. She served on the MSHP Residency, Ambulatory Care and Educational Affairs Committees, and the SMSHP Programming Committee. She was a member of the ASHP Section Advisory Group (SAG) on Preceptor Skills Development and Clinical Leadership, the ASHP 2030 Practice Advisory Panel and the 2022 workgroup to update the ASHP Statement on the Role of the Pharmacist in Emergency Preparedness. Currently, she is Chair of the ASHP Section of Clinical Specialists and Scientists. She is the recipient of the 2021 MSHP Joseph A. Oddis Leadership Award, the 2011 MSHP President’s Award, the 2008 and 2017 SMSHP Pharmacist of the Year Award, and the 2014 HFH Shadow of a Leader Award. She was awarded Fellow Status with ASHP in 2017.
