Accreditation Services Contract Lead Surveyor

Jim Carlson is a lead pharmacy residency surveyor in ASHP's Accreditation Services Division. Jim joined ASHP as a lead surveyor in 2016. Jim earned his Bachelor of Pharmacy degree from Washington State University in Pullman, WA, and his Doctor of Pharmacy from and completed his residency at the University of Minnesota in Minneapolis, MN. Prior to joining ASHP, he was at Regence Blue Cross/Blue Shield in Seattle, where he was Vice President, Clinical Pharmacy Services. Previously, he worked for Group Health Cooperative in Seattle for 30 years in various pharmacy leadership roles in hospital, ambulatory care, and managed care. Prior to his career at Group Health, Jim was on the faculty at the University of Louisiana at Monroe SOP for 2 years. He has received the AMCP Individual Contribution Award, the AMCP Distinguished Service Award and the AMCP Northwest Affiliate Lifetime Achievement Award. Jim has been the program director of two PGY1 Managed Care Pharmacy Residency programs and served as a practitioner surveyor for ASHP for 20 years prior to joining as a lead surveyor. He served as a member of the ASHP Commission on Credentialing as the AMCP representative and served as Chair of the ASHP Commission on Credentialing.
