James M. Hoffman PharmD, MS is a Member in the Department of Pharmacy and Pharmaceutical Sciences and Senior Vice President - Quality and Safety at St. Jude Children’s Research Hospital in Memphis, Tennessee. His career is dedicated to evaluating and improving complex medication use systems and improving patient safety. Hoffman leads quality patient safety efforts across St. Jude where patients with catastrophic diseases routinely require complex care. He focuses on refining existing tools and identifying new means of patient safety event detection, assessing and improving patient safety culture, and developing and improving clinical decision support tools. At St. Jude, he has been involved in efforts related to all of these areas. For example, he collaborated closely with Clinical Informatics leaders to assess and manage alert fatigue in the St. Jude electronic health record and design and implement advanced CDS, including pharmacogenomic CDS. He is dedicated to implementing pharmacogenomics as a patient safety strategy. He is an investigator on St. Jude’s pharmacogenetic implementation effort (PG4KDS www.stjude.org/pg4kds) and provides international leadership through the Clinical Pharmacogenetic Implementation Consortium (CPIC https://cpicpgx.org) where he co-leads CPIC’s informatics working group.
