PGY2 Ambulatory Care residency Louis Stokes Cleveland Veterans Affairs Medical Center, Cleveland OH. Rachel is a primary care clinical pharmacist at the Cleveland Clinic and acts as the Clinical Coordinator for the primary care pharmacy team. Her clinical practice site is the Lakewood Center for Family Medicine, which is home to a family medicine residency where she is a member of faculty and Co-Director of Interprofessional Education. Rachel’s clinical interests include quality improvement, population health, health systems science, transitions of care, and chronic disease state management.
